PhoenixSoul Contract between Parents of Younger Children and Therapist
Upon therapy starting the following information will be sent from your individual therapist:
Confidentiality
As part of my code of practice, I receive regular supervision with another qualified therapist. Should I take aspects of my work with your child to this supervision I will refer to him/her by first name only. My supervisor is bound by the code of ethics regarding confidentiality. It can at times be beneficial to record a session but this would of course need to have your permission first.
Notes may be taken during and after each session which will be kept in accordance with the Data Protection Act 1998. These notes will be securely stored. Please see separate Privacy Policy for General Date Protection Regulations (GDPR) May 2018.
Information which arises in our sessions remains confidential except where I have concern for your child’s or someone else’s safety. In this instance, I may have to consult with another professional but this will be discussed with you first.
If your child would benefit from a referral to other agencies, letters and/or reports may be written at the start and end of our work to inform of progress and your permission would be sought first.
Should I be unable to continue with the work due to abrupt unforeseen circumstances, a qualified colleague will contact you to enquire about your wishes regarding future therapy.
Times and payment
Sessions are £50 per 50 minute sessions. Assessment sessions usually take longer and are charged at £60 for an hour session. I will send an invoice after each session with bank transfer details.
Cancellation policy and endings
Please give 48 hours’ notice of cancellation for no charge to be applied. Naturally life throws unforeseen circumstances our way therefore if cancellation is within the 48 hours then an alternative appointment will be offered but if this cannot be arranged then the full fee will be charged.
Holidays
Please give me advanced notice of any planned holidays so that therapy can be planned accordingly and I will do the same for you. Breaks and holidays will be built into the therapy work with your child as these can often bring anxiety.
Numbers of sessions
Usually at the beginning of the work, it is possible to estimate how many sessions we might be working together for. This is broadly split between short-term work (between 10-14 sessions) and long-term work (25 sessions or more).

Should you need to contact me between sessions please do so in the first instance by email and I will endeavour to get back to you within 24 hours. If you need a quicker response then please do call me and I will call you back as soon as I can.
Remote working
In line with guidance from the ICO (Information Commissioner’s Office), the owner of the device needs to be in agreement for its use in therapy.
By agreeing and signing the contract, you are agreeing to the contents of my Data Protection and Privacy Policy.

Data Retention and Erasure and Portability
Your personal information is stored securely. This is either electronically using password protection, or in paper format in a locked cabinet. Therapeutic notes are stored securely and confidentially for 5 years after the psychotherapeutic process has ended when they will be deleted or destroyed. However, you have the right at any point to ask me to delete any information I hold about you. You also have the right to transfer your information to another party.
Right of access
You have the right to ask for a copy of any personal information and ask me to amend or change any incorrect information.
Sharing of information
No information will be shared with a third party unless for the reasons stated in the contract. Breaking confidentiality may include in the unusual circumstances of; needing to be compliant with a court order, concerns over child protection, acts of terrorism or drug trafficking, or information regarding fraud.
